Your First Visit |

Please arrive 15 minutes before your scheduled appointment time in order to complete your new patient paperwork. Your initial appointment will last approximately 90 minutes. Follow up appointments generally last between 45 and 90 minutes depending on your treatment needs. |

Things to remember |
| •Eat something at least two hours before getting acupuncture. Don’t be hungry but don’t be overfull. •If possible, wear loose and comfortable clothing. •Don’t drink alcoholic beverages before your appointment. •The day before your appointment, please refrain from brushing or scraping your tongue. Your tongue coating is an important element in tongue diagnosis. •As a courtesy to our clients with chemical sensitivities, please refrain from wearing scented products to the clinic. |

Fees |

Initial Appointment: $150 Payment is due in full at your scheduled appointment. We accept cash, check and credit cards. I accept some forms of insurance. Please call or email Elisa to verify if you have acupuncture benefits. If I do not accept your insurance, a superbill can be provided upon request. |
Canceled or Rescheduled Appointments |

If you need to cancel or reschedule your appointment time, please give a minimum of 24 hours notice to avoid the cancellation fee. Please note, if you are more than 20 minutes late to your appointment, this may be considered a canceled appointment and subject to the full cancellation fee. |
